Questi enti fanno da controllori, e le agenzie farmaceutiche sono le controllate. Un possibile conflitto nasce se i controllati diventano poi i controllori, o viceversa i controllori passano a ruoli importanti nelle agenzie di cui sopra. Non un'ipotesi remota. Patrizia Cavazzoni è recentemente approdata nella Pfizer come CMO, dopo essere stata alto funzionario della FDA (Food and Drug Administration).
Stephen Hahn è passato alla Flagship Pioneering, compagnia sostenitrice di Moderna, dopo essere stato Commissario della FDA durante la prima fase della pandemia. Diversi quotidiani negli States hanno denunciato il fenomeno delle "revolving door", segnalando una mancanza di trasparenza.

Vediamo questo sistema delle revolving doors a tutti i livelli, sia per quanto riguarda questo passaggio in entrare e in uscita dalle case farmaceutiche e dalle agenzie del farmaco, ma anche ai massimi livelli della politica".
Ora negli USA però tira aria di cambiamento, promessa dal nuovo segretario della salute, Robert Kennedy Jr.
