FSH, hipofiz bezinden salgılanır ve testiste özellikle Sertoli hücreleri üzerinden sperm üretiminin desteklenmesinde rol oynar. Testisteki sperm üretimi ile hipofiz arasında ise inhibin B'nin de yer aldığı bir geri bildirim sistemi vardır.
Sperm üretimi azaldığında FSH yükselebilir. Ancak önemli nokta şudur: FSH'nin yüksek olması sperm bulunmadığını kesin olarak göstermez; FSH'nin normal olması da sperm üretiminin mutlaka normal olduğu anlamına gelmez.
Bu nedenle erkek infertilitesinde yalnızca FSH değerine bakmak yeterli değildir. Videoda FSH, LH, testosteron ve spermiogramın birlikte değerlendirilmesinin neden önemli olduğunu anlatıyorum.
FSH yüksek, LH ve testosteron normal olduğunda sorun daha çok sperm üretimiyle ilişkili testis fonksiyonlarında olabilir. FSH ve LH birlikte yüksek, testosteron da düşükse testisin daha geniş kapsamlı fonksiyon kaybı açısından değerlendirilmesi gerekir.
