Berdasarkan informasi di lapangan, korban tewas bernama, Asep Kohar (51), Marsidi (34), Nana Mulyana (42), Husni Saepul (44) warga Dusun Bakanlio, Desa Karyasari, Kecamatan Rengasdengklok. Agus Mulyana dalam kondisi kritis.
Salah satu saksi, Wawan Sutisna menyebut kejadian keracunan di area produksi PT Multidaya Putra Sejahtrera di Dusun Babakan Lio, Desa Karyasari, Kecamatan Rengasdengklok, Kabupaten Karawang itu terjadi sekitar pukul 09.00 WIB.
