Kalpana Chawla’s story is one of extraordinary determination—born in Karnal, India, she grew up fascinated by the sky and went on to become the first Indian-born woman in space, flying on the Space Shuttle Columbia in 1997 and again on its final mission STS-107 in 2003. This video explores her inspiring biography, her education in aeronautical and aerospace engineering, and her work in microgravity research, before turning to the tragic Columbia disaster. Just 82 seconds after launch on January 16, 2003, a suitcase-sized piece of foam from the external tank broke off and struck the shuttle’s left wing, breaching the Reinforced Carbon-Carbon (RCC) panels that protected it from re-entry heat. Engineers raised concerns during the mission, but NASA managers—used to seeing foam loss in earlier flights—dismissed it as unlikely to cause catastrophic damage, and crucial requests for Department of Defense imaging were never followed through. In reality, the hole in the wing allowed superheated plasma to enter during re-entry on February 1, tearing Columbia apart and killing all seven astronauts, including Kalpana. Why didn’t NASA save them? Because at that time, there were no certified repair techniques for RCC panels, no EVA tools on board to inspect or patch the wing, no way for Columbia to reach the International Space Station, and no rescue shuttle prepared for rapid launch—the only possible rescue scenario would have required unprecedented emergency measures that were never triggered. The Columbia Accident Investigation Board later concluded that both technical limitations and organizational culture failures doomed the crew.
