Ali, Nazlı’yla geçirdikleri lunapark gecesini Ferman’la paylaşmak ister ama Ferman, Ali’nin teklifsiz samimiyetinden rahatsız olur; bir de üstüne asistanları kaytarırken yakalayınca herkesi sınav yapmaya karar verir. Diğer tarafta daha yeni ayrılmış yapışık ikizlerin durumu süratle kötüye gitmektedir. Asistanlar bir yandan Ferman’ın sınavına hazırlanır bir yandan ikizlerin durumuna çözüm bulmaya çalışırken Cerrahi Bölüm’ün maskotu, sekiz yaşındaki Betüş, hastaneye belki yirminci yatışını yapar. Ali’yle aralarında çabucak bir yakınlık doğar ve Ali onun ameliyatını bizzat yapmak ister. Ama önce Ferman’ın onun için özellikle zorlaştırdığı sınavı geçmelidir. Sonunda ikizler vakasında bütün çareler tükenir ve ekip korkunç bir seçimle karşı karşıya gelir: Bir kardeşin yaşaması için diğerinin ölmesi gerekir. Ama hangisi? Kim bu ölümcül kararı verebilir?
Ali, taşra bir şehirde büyümüş yoksul bir ailenin oğludur. Sahip olduğu otizm ve savant sendromundan dolayı çevresi tarafından sürekli dışlanmış, ötekileştirilmiştir. İletişim kurmakta zorluk çeken Ali'nin hayatında iki dostu vardır. Biri abisi, ikincisi de tavşanı. İkisini de sırayla kaybeden Ali, tek bir temennide bulunur: İnsanlar ölmesin. Abisinin ölümünden sonra babası tarafından evlatlıktan reddedilen Ali, yetimhanede büyür. Savant sendromu nedeniyle tıp konusundaki yetenekleri muaazzam olan Ali'yi keşfeden Doktor Adil, Ali'ye göz kulak olur. Tıp fakültesini kazanan ve birincilikle bitiren Ali, Doktor Adil'in başhekim olduğu hastanede asistan cerrah olarak işe başlar. Hastane yönetimindeki birkaç kişi Ali'nin hastalığı nedeniyle bu işe uygun olmadığını söylese de Doktor Adil, Ali'nin arkasında durarak onu işe aldırır. Ali, hastanede geçirdiği süre içinde etrafındaki herkesi değiştirecektir.
