Incident Investigation: Key Fob Triggers Explosion in Service Vehicle | WorkSafeBC
This video recreates an incident in B.C. where a service van exploded. The van contained an oxyacetylene welding system, including compressed acetylene and gasoline, and flammable gas leaked and built up in the van overnight. The investigation concluded that the gas was ignited by a spark in the electrical system triggered by a key fob, causing an explosion that destroyed the van.Although no one was injured in this incident, it had the potential to cause serious harm. This video illustrates the elements necessary to create an explosion (fuel, oxygen, and an ignition source), and the safety deficiencies that led to the incident. It also explains why proper storage, safe work procedures, and adequate ventilation are all critical when transporting or storing flammable materials.
